You must bring your child’s birth certificate, registration of birth, or baptismal certificate (if the date of birth is included on the certificate).
If your child’s name is different than the one on the birth certificate, you must bring legal documentation providing the legality of the present last name. Adoption papers and court orders are acceptable: a letter from an attorney is not.
You must bring your child’s immunization records. Regulations of the Pennsylvania State Department of Health require that children be immunized prior to admission to school for the first time.
The requirements are:
- 4 doses of tetanus (1 dose on or after the 4th birthday)
- 4 doses of diphtheria (1 dose on or after the 4th birthday)
- 4 doses of polio
- 2 doses of measles
- 2 doses of mumps
- 2 doses of rubella (German measles)
- 3 doses of hepatitis B
- 2 doses of varicella (chickenpox) vaccine or history of disease
If there is a medical or religious reason why immunizations are not complete, please contact your school nurse. You should bring any information concerning your child’s health history. This information should include all childhood diseases and operations. (Bring along your baby book.)
District policy also requires that you bring three (3) items as proof of residency. Examples of acceptable proofs of residence include, but are not limited to the parent’s/guardian’s current:
One of the Following:
- Current deed, lease, or mortgage statement (statement must be no more than 6 weeks old at time of enrollment)
AND
Two of the Below:
- Valid driver’s license or PA Department of Transportation identification card
- Current utility bill
- Property tax statement or bill
- Check stubs from wages, public assistance, or Social Security
- All other verification documents submitted will be considered on an individual basis.
