Preseason Info

 
To participate in athletics at Peters Township, student-athletes must complete and submit the following:
 

1. Register on Family ID

All athletes must be registered on Family ID.  The District uses Family ID to provide online sports registration for students. Once completed, families can access this information to simply update and resubmit the information.  Visit the Peters Township portal on FamilyID.com to create an account and register for Peters Township athletics.

For multi-sport athletes, you will have to sign up for each additional sport prior to the season.

2. Sports Physicals

For the 2024-25 school year, all participants must complete a physical no earlier than May 1, 2024 and submit the PIAA Physical Forms to their FamilyID registration at least ten days prior to the first practice date.  Your assistance is appreciated!

All physical paperwork must be uploaded into FamilyID.  The athletic office does not accept paper copies.

For Winter/Spring Sports ONLY.  This form must be completed within six weeks prior to start of winter and/or spring season.
This form is only for additional seasons during the same school year.  (Ex: playing a fall sport, then a winter sport).

For Winter/Spring Sports ONLY.  This form must be completed within six weeks prior to start of winter and/or spring season.
This form must be completed if a major injury occurred since last physical (marked as YES on Section 7).

For Wrestlers ONLY

3. Concussion Baseline Testing

All student-athletes must complete a concussion baseline test every two years.  These tests are offered by our athletic training staff to those in Grades 7, 9, and 11, and if the athlete did not complete a baseline test in the previous year.

Questions regarding baseline testing can be directed to the Peters Township Athletic Training Office at (724) 941-6250 x 8804.  They will respond to any voicemail messages as soon as they can.  Thank you.

4. Submit Athletic & Activity Fee Form

Please complete the Athletic and Activity Fee form below and submit it with payment to the athletic office.  All athletic fees must be paid by end of tryouts.
 

5. Concussion Awareness

As required in Peters Township School District Policy, the Peters Township Athletic Department offers parents and student athletes a information on baseline ImPACT testing, updated knowledge about concussions as a mild traumatic brain injury (TBI), treatment, management, and common signs and symptoms of a concussion. All parents, coaches, and student athletes are strongly encouraged to attend this informational meeting.

Annually, prior to trying out for a sport, all parents and student athletes will be required to participate in and document having successfully completed either option A or B below, in order for the athlete to practice or play:

Option A

Review all information pertaining to concussions provided by the School District and provided below. Parents and student athletes must also review the video provided of  the annual preseason informational concussion meetings. 



Option B

Take an approved educational course on concussions. Links to these courses are provided below.
