2016-2017 Sports Physical Information
The
spring sports season begins on Monday March 6, 2017. If you have not
participated in a previous season sport, the following forms are to be turned
into the athletic office no later than Friday February 24, 2017: PIAA CIPPE
forms Sections 5 (Health History) & 6 (Physical Form) along with Section 7
(recertification if a physical is dated before January 23, 2016); For students
who have participated in a previous season sport, Section 7 is the only form
due to our office by February 24th.
