McMurray/PTMS Student Walker Forms
Attn PTMS Walkers: This form must be completed for all students who opt not to use District Transportation and walk after school. Students must hand this signed permission form into Mrs. B in the main office in the morning if they are walking home instead of taking District transportation. Parents: If your child is planning on being a permanent walker for the school year, the main office needs a written permission form from parents stating that your child will be a permanent walker for the school year. Your child's name will then appear on our permanent walker list. If you have questions regarding our Student Walker Form or about being a permanent walker, please contact Mrs. Bobrzynski in the PTMS Main Office at (724)941-2688 ext. 4243.

Please submit this completed form to the Principal's office in the school that the child attends.
*Educational trips will not be approved during the administration of standardized tests (PSSA or Stanford) or semester exams. See the testing calendar on the main district page under "Calendar Quick Links" (right column under the Calendar).
