Preseason Info

 
To participate in athletics at Peters Township, student-athletes must complete and submit the following:
 

1. Register on Family ID

All athletes must be registered on Family ID prior to submitting a physical.  The District uses Family ID to provide online sports registration for students. Once completed, families can access this information annually to simply update and resubmit the information.

2. Sports Physicals

All participants must complete a physical no earlier than June 1, 2018 and return the PIAA Physical Forms to the high school athletic office at least ten days prior to the tryout date.  
 
Section 5 (Health History) & Section 6 (Physical Form) should be dated no earlier than June 1.  Note: Please indicate the sport the student will be trying out for on Section 6.
 
All physical paperwork must be uploaded into FamilyID.  The athletic office is no longer accepting paper copies.  
 
Section 7 (if necessary) must be completed if a physical (Section 6) is dated more than six weeks prior to the start of the season.  This indicates no changes have occurred since the physical was completed.   
 
Section 8 (if necessary) must be completed for any student who, subsequent to completion of Sections 1 through 6 of this CIPPE Form, required medical treatment from a licensed physician of medicine or osteopathic medicine.
  
Again, all forms must be uploaded into FamilyID .  The athletic office is no longer accepting paper copies.
 
PIAA CIPPE forms Sections 5 (Health History) and Section 6 (Physical Form) must be dated after June 1, 2018 and must be uploaded into FamilyIDRegistration on Family ID must also be completed.
 

3. Concussion Baseline Testing

All student-athletes must complete a concussion baseline test every two years.  These tests are offered by our athletic training staff to those in Grades 7, 9, and 11, and if the athlete did not complete a baseline test in the previous year.
 
Any questions regarding baseline testing can be directed to the Peters Township Athletic Training Office at (724) 941-6250 x 5607 after 2:00 pm, Monday through Friday.
 

4. Submit Athletic & Activity Fee Form

Please complete the Athletic and Activity Fee form below and submit it with payment to the athletic office.  The activity fee is due by the end of the first week of practice.
 
 

5. Concussion Awareness

As required in Peters Township School District Policy, the Peters Township Athletic Department will offer parents and student athletes a preseason informational meeting on baseline ImPACT testing, updated knowledge about concussions as a mild traumatic brain injury (TBI), treatment, management, and common signs and symptoms of a concussion. All parents, coaches, and student athletes are strongly encouraged to attend this informational meeting.

Annually, prior to trying out for a sport, all parents and student athletes will be required to participate in and document having successfully completed either option A or B below, in order for the athlete to practice or play:

Option A

Review all information pertaining to concussions provided by the School District and provided below. Parents and student athletes must also review the video provided of  the annual preseason informational concussion meetings. A link to the meeting video can be found below.  

Option B

Take an approved educational course on concussions. Links to these courses are provided below.
